Accident & injury reporting for students

Accident and injury reporting and investigation process:​​​

Injury/Illness Investigation Form - worker's report of injury or occupational disease to employer, is to be completed by College students on unpaid practicum or experiential learning work related accidents, injuries or incidents, in British Columbia, including near misses.

 must be completed by the First Aid Attendant, the Student, or the Student's Instructor and forwarded to Human Resources within one day of the injury or incident.

  1. Injured/ill person reports to First Aid; 
  2. First aid (Security) completes an Incident Investigation Form; submits the form to Safety, Security & Risk Management (SSRM);
  3. SSRM sends the form to the injured/ill person's supervisor; and
  4. The completed form is sent back to SSRM.
